I, in consideration for attendance and participation in the University of Sidekicks Martial Arts Training and Fitness, acknowledge the existence of certain inherent risks in this type of training. I (we) understand injuries may occur and authorize an agent from the University of Sidekicks to obtain medical treatment for myself (my child) in the event I am unable to. I further relieve costs, claims and liabilities of any kind arriving out of the use of the facility, and any activities I (we) participate in . As a consideration for my visiting the facility, I assume any risk of damage or loss of property , or injury to myself (my child). I also understand that an agent of the University of Sidekicks may photograph and/or videotape classes for the purpose of display, training, and advertising and I give the University of Sidekicks my permission to use myself (my child) in such pictures if necessary, for no financial gain. I also hereby state that the I (we) are physically fit to take part in class and do so of our own free will for agreed upon fee. I understand there is no refund policy on any monies I will pay to this academy.
